The Bridges to Health (B2H) program services include family and caregiver supports, skill building, day habilitation, education, prevocational support, supported employment, respite services, crisis response, intensive in-home supports, adaptive and assistive equipment and accessibility modifications. The services will follow the child through the foster care system, to home or into adoption, to lower levels of foster care, and to adulthood up to age 21 as long as the child continues to meet waiver eligibility requirements. Only children in foster care will be eligible for B2H services.

The B2H services will be provided through Health Care Integration Agencies and Local Departments of Social Services to provide coordinated care for the enrollees and manage the provider network. Services will be billed on a fee-for-service basis and reimbursed by Medicaid.
