ShareThe Community Living Program: Supporting Independence & Choice in the Community -- Program Announcement & Grant Application Instructions

This program announcement provides an opportunity for the state units on aging to modernize their approaches to service delivery and to prioritize helping individuals who are at imminent risk of nursing home placement but not eligible for Medicaid to avoid nursing home placement and spend-down to Medicaid, consistent with the long-term care provisions that were included in the 2006 Amendments to the Older Americans Act.

Under this announcement, the Administration on Aging will award Cooperative Agreements to assist State Units on Aging (SUA) to develop Community Living Programs (formerly known as Nursing Home Diversion Programs). A successful SUA applicant will propose to implement projects in partnership with Area Agencies on Aging, Aging and Disability Resource Centers (ADRC) or ADRC type providers, aging services provider organizations, and other long-term care stakeholders including the Single State Medicaid Agency that strengthen the capacity of the Aging Network to target and serve individuals at highest risk of nursing home placement and spend-down to Medicaid with flexible services, including offering consumer directed options.
