Provider Agreement From the Pennsylvania's Department of Public Welfare, Office of Developmental Programs

Pennsylvania's Department of Public Welfare (DPW) finalized a new Provider Agreement for Participation in Pennsylvania's Consolidated and Person/Family Directed Support Waivers. The new agreement will serve as the statewide agreement between waiver providers and DPW, as the state Medicaid agency for the Pennsylvania developmental disabilities system. The agreement went into immediate effect on July 1, 2009.
