United States v. State of Texas Monitoring Team Report for Lubbock State Supported Living Center

Since January 2010, compliance monitoring teams approved by the United States Department of Justice (DOJ) and Texas Department of Aging and Disability Services (DADS) have been conducting baseline reviews. At the Lubbock State Supported Living Center, the monitoring team found serious staffing problems. They also were concerned that inappropriate groupings of people in smaller settings may have created problems and conflict. Additional concerns included inadequate risk assessments, overuse of certain broad diagnoses, inconsistent data collection and reporting, and inconsistent implementation of assessment results into individualized plans.
