A Strategic Framework 2010?2015 Optimum Health & Quality of Life for Individuals with Multiple Chronic Conditions

The federal Department of Health and Human Services (HHS) Interagency Workgroup on Multiple Chronic Conditions released a draft Framework on Multiple Chronic Conditions which was created to improve care for people with multiple co-occurring chronic conditions. The draft was sent out with a request for comments from behavioral health stakeholders to be sure that the issues were adequately addressed. Some of the objectives of the draft included better tools and information sharing for workers who deliver services for people with multiple chronic conditions, research to understand care coordination and management better, and public health system changes.
