Medicare Special Focus Facility Initiative Participants

This document lists the names of nursing homes in the Medicare Special Focus Facility (SFF) Program to improve the quality of care. Facilities in the program are selected by the states from a list nursing homes in the state with the lowest quality of care as documented by annual inspections or investigations following consumer complaints. The program lasts for about 18 months. The list includes the progress/outcomes for each nursing home in the SFF Program.
