The demonstration project was designed to determine whether the program could reduce unnecessary referrals to specialty care by primary care physicians or nurse practitioners and promote better management of chronic illnesses at the primary care site. The demonstration project built on other models of care management that emphasize joint decision-making and collaboration among providers.
