Request for Proposals (RFP) & Application for Provider Practices to Participate in the Health Home (Medical Home) Pilot Program RFP No. 529-10-0057
The request for proposal was released on April 5, 2010. Proposals are due July 2, 2010 by 3:00 PM CST. The Texas Health and Human Services Commission (HHSC)
seeks health home pilot projects to increase access to primary care and specialty services for children receiving Medicaid benefits, improve the quality of
primary care and specialty services for children enrolled in Texas Medicaid, meet the first two objectives in a cost-effective manner, and provide care in a
patient/family-centered manner. HHSC will review and evaluate the prospective care practices based upon six core competencies, its
philosophy, core competency emphasis, practice size, geographic location, information technology infrastructure, staffing, population served, and other
variables. The six core competencies include patient access, quality improvement, patient/family centeredness, population approach to care, coordinated and
clinically managed care, and team-based, comprehensive care. Up to eight health home pilot projects will be selected for a period of
24 months. Each pilot project must include a minimum of 5000 children enrolled in Medicaid under 21 years of age.
