Service Areas
The Department of
Health and Family Services is issuing this document in order to
solicit proposals to contract as a care management organization
under the Health and Community Services Contract to provide the
Family Care benefit. Family Care is an innovative way of
delivering long-term care services. It was designed with the input
of consumers, advocates, providers, State and local officials, and
others in order to address concerns about the cost and complexity
of the long-term care system, inequities in availability of
community and institutional services, and projections of an aging
populations growing demand for long-term care. It is intended
to provide cost-effective, comprehensive and flexible long-term
care that will foster consumers independence and quality of
life, while recognizing the need for interdependence and support.
