Voluntary Chronic Care Improvement Under Traditional
Fee-for-Service Medicare
This notice
informs interested parties of an opportunity to apply to implement
and operate a chronic care improvement program as part of Phase I
(CCI-I) of the Voluntary Chronic Care Improvement Under
Traditional Fee-for-Service (FFS) Medicare initiative as
authorized by section 721 of the Medicare Prescription Drug,
Improvement, and Modernization Act of 2003 (Pub. L. 108-173).
Organizations eligible to apply to
implement and operate chronic care
improvement programs under CCI-I
include: (1) disease management organizations; (2)
health insurers; (3) integrated delivery systems; (4)
physician group practices; (5) a consortium of entities; or
(6) any other legal entity that meets the requirements of this
notice.
