ShareMoving Individuals With Mental Illness Out of the Institutional Setting of Nursing Homes (including IMDs) & Into the Community-Based Option of Supportive Housing: A Plan to Rebalance and Change the Illinois System

The Supportive Housing Providers Association in Illinois submitted this proposal to the Illinois Nursing Home Safety Task Force. It describes how the state could move individuals with mental illness out of nursing homes and other congregate care settings into community-based supportive housing. The plan proposes to rebalance and change the Illinois system for serving individuals with mental illness. The proposal discusses funding, community capacity, and a transition timeline.
