District of Columbia Special Education Case Management Pilot: Draft Scope
of Work
The case management program will provide training, deployment, and
management for case managers whose primary role will be to improve
service delivery to students with disabilities through partnerships with
the students, their families, and individualized education program (IEP)
teams. Services are projected to start April 1, 2008.
The job of the case managers will be:

Build a trusting relationship with each student
on the case manager's case load and the student's family, including
making home visits.

Facilitate implementation of the student's
individual education plan or hearing officer determination (HOD)

Advocating for the student's educational and
related service to be identified and met

Encouraging creative strategies for meeting the
student's needs

Facilitating agreements between the Multi
Disciplinary Team and the student's family concerning the special
education and related services the student will receive

Collect data on deliverables, which could
include rates of placement, number of HODs, amount of services received
by students, etc
