 Medical Home Scan
Between January and October of 2008 National Academy of State Health Policy staff conducted a scan of state Medicaid and
State Children's Health Insurance Plan (SCHIP) efforts to advance medical homes. A variety of efforts were identified. Some had implementation dates in the 1990s. Of these, a total of 34 initiatives in 31 states were identified as recent state efforts to advance medical homes for Medicaid or SCHIP program participants.
