 Request For Proposals: Disease Management Services for Medicaid Aged, Blind and Disabled Members
The Georgia Department of Community Health (hereinafter referred to as "DCH") is requesting proposals for Disease Management Services for the Aged, Blind and Disabled (hereinafter referred to as "ABD") Medicaid populations. The objective of this procurement is to obtain the services of qualified, experienced, accredited, and financially sound disease management organizations (hereinafter referred to as "DMOs") to develop and administer a disease management program in one of two regions of the state.
