 Florida Medicaid Disease Management Program
The Florida Agency for Health Care Administration issued an
Invitation to Negotiate (AHCA ITN 0612) seeking a contractor for
the Florida Medicaid Disease Management Program. It involves a
contract with 10 hospitals and McKesson Health Solutions
(providing call center services) for statewide disease management
for diabetes, hypertension and asthma, statewide and
congestive heart failure in South Florida. Pfizer Health Solutions
is under contract to provide care management software and
support for this program. Lifemasters holds a contract to manage
congestive heart failure in North Florida through August 31,
2006. People being served through that contract will not be eligible
for services under any contract resulting from this ITN
until September 1, 2006. Proposals were due July 24, 2006.
