 Telehealth Pilot Program For Chronic Conditions
The Colorado Department of Health Care Policy and Financing (DHCPF)
seeks to create a pilot program to demonstrate the feasibility of
delivering health care services through telemedicine when managing
chronic diseases such as heart failure, diabetes, chronic
obstructive pulmonary disease, and combined conditions. Proposals
are due March 5, 2007. The one-year contract period is planned to
begin on July 1, 2007. The contract will have four additional
one-year renewal periods. The maximum value for the first year of
the contract period will be met by monthly payments of $31,744 and
will not exceed $380,928 in total. No additional funds are available
if enrollment exceeds the proposed number of clients. Any necessary
home monitoring equipment and devices must be supplied and
maintained by the contractor.
