February 2007 Telehealth Pilot Program For Chronic Conditions The Colorado Department of Health Care Policy and Financing (DHCPF) seeks to create a pilot program to demonstrate the feasibility of delivering health care services through telemedicine when managing chronic diseases such as heart failure, diabetes, chronic obstructive pulmonary disease, and combined conditions. Proposals are due March 5, 2007. The one-year contract period is planned to begin on July 1, 2007. The contract will have four additional one-year renewal periods.
