ShareExploring & Implementing Person Centered Approaches

The New South Wales, Australia, Department of Ageing, Disability and Home Care released the document to guide on person-centered planning and approaches for program service providers in New South Wales. The guide is not a manual of how to implement person-centered programs, rather it is intended to help providers and provider organizations understand the concepts of person-centered planning, inform service providers about fundamental strategies and practices to meet the principles and key result areas of a NSW Community Participation Program, and inform service providers of fundamental strategies and practices of effective person-centered planning. The document is the work of a consortium comprising Australian Catholic University (ACU), Ellis MacRae & Associates and Sherwin & Associates.
