 Application for Proposal Submitted in Response to RFP
This document describes the scope of the Ohio
Department of Mental Retardation and Developmental Disabilities request
for proposals for an organization to run the Fairfield Center
Intermediate Care Facility for Mental Retardation. Proposals are due by
February 15, 2008. The proposals can address one of two options:
acquisition of the license only or acquisition of the license and
purchase of the property. All applicants must agree to reduce the
licensed and certified capacity of the facility to no more than 48
individuals by June 30, 2011. To reach this goal, applicants must agree
to help stakeholders develop a plan to aid the downsizing the facility
and transitioning of individuals to community placements.
